Course accreditation forms and templates
All forms required to apply for accreditation of a new course, as well as to renew or amend existing courses, can be downloaded from this page.
Please be aware that additional supporting documentation is also required in each case. Further details of the submission processes for each type of application are provided on this site.

FM-ACC1 Application for accreditation of a new course [DOC 238kb]. Form FM-ACC1 must accompany an application to accredit a new course in Queensland.

FM-ACC2 Application for amendment to an existing accredited course [DOC 289kb]. Form FM-ACC2 must be completed when making amendments to a previously accredited course. This may occur when training packages containing national competency standards relevant to an accredited course are revised or endorsed. In this case, the course developer has 12 months from the date of publication to amend the course to incorporate the new competency requirements.

FM-ACC3-Ext Application for renewal (extension) of an accredited course - without amendment [DOC 267kb]. Form FM-ACC3-Ext must be completed where a course is being renewed for a short period of time. Extensions without amendment are usually only granted where the training needs of an industry sector will be covered by a training package, or revisions to an existing package, in the near future.

FM-ACC3-Reaccred Application for reaccreditation of an existing accredited course [DOC 251kb]. Most courses presented for reaccreditation require some updating. Form FM-ACC3-Reaccred should be used in these cases. Requirements for reaccreditation are different to those requirements necessary for an extension, as the course must be reviewed and updated to meet contemporary training needs, and the period of reaccreditation can be granted for up to five years.

FM-ACC4 Course concept proposal [DOC 287kb]. Form FM-ACC4 is required to request in-principle support for the proposed course, prior to actually proceeding with the development. This form should demonstrate an industry or community need for the training, and evidence of research.

FM-ACC5 Course development advisory committee (CDAC) - endorsement declaration [DOC 180kb]. Form FM-ACC5 is required to validate the membership of the course development advisory committee. The form is submitted to the accreditation team for in-principle support to ensure that the appropriate industry and peak bodies have been involved in the development of a course. Once the course concept is approved in principle, this form must be signed by all committee members.

FM-ACC6 Conflict of interest and confidentiality declaration for course development advisory committee (CDAC) members [PDF 49kb]. Each member of the course development advisory committee is required to complete Form FM-ACC6, advising of any possible conflicts of interest, to ensure that the course development process is unbiased and objective.

Monitoring and Evaluation Template [DOC 169kb]. An optional monitoring and evluation report template has been developed by the Accreditation Team to assist with this accreditation requirement.
