The following vocational placement forms and templates are provided for download. Note that these documents are in PDF (Portable Document Format) and need Adobe Acrobat Reader to be opened. Adobe Acrobat Reader is freely available from Adobe.
Form VPS01 - Application for recognition of a vocational placement scheme [DOC 224kb]
This form must be used to apply for a vocational placement scheme to be approved. The application should also include a sample training plan.
Form ACC 20 - High risk vocational placement scheme reporting template[DOC 164 kb]
This form must be used to report instances where a vocational placement scheme is rated as high risk. You will be advised in the vocational placement scheme approval information notice when you need to submit this template.
Vocational placement agreement template [DOC 166kb]
Once a placement is approved, a vocational placement agreement must be completed before students can be placed with an employer.
Sample training plan [PDF 109kb]
An example of a sample training plan. A training plan is required for each learner who signs a vocational placement agreement.
