Deed of Variation Form [Word 94kb]
This Deed of Variation form is required from the supervising registered training organisation when requesting any variation to its 2004-06 User Choice Program contract. Contact your contract manager if you require assistance when completing this form. (Please note that variations may only occur in line with each supervising registered organisation's contract. Also, no variation is binding unless it is in writing and signed by both parties.)
Eligibility Form (Priority 4 Rating Traineeship) [Word 159kb]
If a supervising registered training organisation has advised an employer and a trainee that it can accommodate the trainee under its 2004-2006 User Choice contract for a priority 4 rating traineeship the supervising registered training organisation must retain evidence that the trainee is a member of one or more of the Priority Population Groups. Completing and retaining this form and attaching other evidence where necessary is considered sufficient written evidence.
Notification of Partnering [Word 144kb]
Written notification to be submitted to the Department at least 21 days prior to the commencement of the provision of the services by the partnering organisation.
Statement of Disclosure [Word 137kb]
Information to be provided to the employer and apprentice or trainee by the supervising registered training organisation for the 2004-06 User Choice Program.
