Major amendment of registered training contract [PDF 46kb]
The 'Major amendment of registered training contract' form is used where the parties to a registered training contract wish to apply to the Queensland Department of Education, Training and the Arts to amend their contract. Examples of major amendments include changes to:
- the start date of the apprenticeship or traineeship
- employment status - e.g. from part-time or school-based to full-time arrangements
- change of qualification level - e.g. from Certificate II in Retail Operations to Certificate III in Retail Operations
- date of birth - where date of birth impacts on legislated age restrictions
- level of education - where minimum level of education is an issue
- number of qualified persons - where the allowable ratio of apprentices/trainees to qualified persons/tradespersons is exceeded.
Minor amendment of registered training contract [PDF 23kb]
The 'Minor amendment of registered training contract' form may be used by a party to a registered training contract to notify the Queensland Department of Education, Training and the Arts of a minor amendment to their registered training contract. Examples of minor amendments include:
- change of an apprentice's or trainee's name - e.g. through marriage or by deed poll
- change of a party's address
- correction of a typographical error in a party's name or address
To notify the department that the supervising registered training organisation has been replaced, please use the "Change of supervising registered training organisation" form. For a change of qualification resulting from the original qualification being replaced by a training package, please use the "Agreement to transition" form.
