A
Advance claim form [PDF 47kb]
The 'Advance claim form' (Apprentice and Trainee Land Travel and Accommodation Subsidy) is used by apprentices/trainees who are experiencing financial difficulties when attending off-the-job training with their training provider and would like to claim a travel and/or accommodation subsidy. This form is also used when an apprentice or trainee needs payment in advance of the standard or special travel and accommodation subsidy.
Agreement to transition [DOC 175kb]
The 'Agreement to transition' form is used where an employer and their apprentice or trainee wish to apply to the Queensland Department of Education, Training and the Arts to change the qualification in which the apprentice or trainee is being trained. The new qualification must be at the same Australian Qualifications Framework level, or be aligned to the apprentice's or trainee's original calling.
Amendment of school-based training contract [PDF 33kb]
The 'Amendment of school-based training contract' form is used by the parties to a training contract to notify the Queensland Department of Education, Training and the Arts of a change in apprenticeship or traineeship arrangements, such as a change from school-based to full-time or part-time arrangements. The parties to a school-based training contract must notify the department when the apprentice or trainee leaves school.
Application for Air Travel [DOC 297kb]
Apprentices/trainees should use the Application for Air Travel if they meet the following criteria:

Apprentices/trainees reside in Cape York Peninsula or Islands of the Torres Strait

Apprentices/trainees who are required to travel 1400 km or more (return) to the location of the training provider
Application for amendment of probationary period [PDF 74kb]
The 'Application for amendment of probationary period' form may be used by an employer and their apprentice or trainee to apply to the Queensland Department of Education, Training and the Arts for an extension or reduction of the probationary period of their apprenticeship or traineeship. The application must be made before the original probationary period expires.
Apprentice and trainee travel and accommodation subsidy claim form (other than school-based apprentices and trainees) [PDF 59kb]
Apprentices and trainees should use the 'Apprentice and trainee travel and accommodation subsidy claim' form to claim travel and accommodation subsidies when attending off-the-job training with their training provider. To be eligible, the distance travelled must be more than 100 kilometres round-trip. This form should not be used by school-based apprentices and trainees. (Assistance is available for school-based apprentices and trainees from the schooling sectors and application forms are available from schools.)
Assignment (transfer) of registered training contract/change of legal name [PDF 55kb]
The 'Assignment (transfer) of registered training contract/change of legal name' form is used by the parties to a registered training contract to notify the Queensland Department of Education, Training and the Arts of any of the following changes to their training contract:
- sale of the employer's business
- dissolution of a partnership (where the employer is in partnership)
- a change in the legal name of the employer.

Cancellation of a training contract [PDF 49kb]
The 'Cancellation of a training contract' form is used by the parties to a training contract to notify the Queensland Department of Education, Training and the Arts that they have agreed to cancel the contract.
Change of supervising registered training organisation [PDF 59kb]
The 'Change of supervising registered training organisation' form should be used by registered training organisations, employers, and apprentices or trainees, where the supervising registered training organisation is changed.
Completion of apprenticeship or traineeship - notice of issue of qualification [DOC 152kb]
The 'Completion of apprenticeship or traineeship - notice of issue of qualification' form must be completed by the supervising registered training organisation. The completed form must be sent to the Queensland Department of Education, Training and the Arts when an apprentice or trainee has achieved all competencies required in their training plan and a qualification has been issued by the training organisation.

Employer resource assessment [PDF 113kb]
The 'Employer resource assessment' form is completed and retained by supervising registered training organisations to verify that the employer of an apprentice or trainee is able to provide, or arrange to provide, the facilities, range of work, supervision and training required under the apprentice's/trainee's training plan.
Extension of nominal term of an apprenticeship or traineeship [DOC 159kb]
The 'Extension of nominal term of an apprenticeship or traineeship' form may be used by the parties to a registered training contract to apply for an extension of the term of the contract. The parties may seek an extension if the apprentice or trainee will not complete all required training by the nominal completion date of the apprenticeship or traineeship.

Format of advice from Queensland Racing to DET [PDF 17kb]
The 'Format of advice from Queensland Racing to DET' form should be used by the Queensland Racing to advise the Queensland Department of Education, Training and the Arts (DETA) of the amendment, completion or cancellation of a training contract of an apprentice or trainee in their employ.
Funding assistance for apprentices and trainees application form [PDF 32kb]
The 'Funding assistance for apprentices and trainees application' form is used by apprentices and trainees whose training contracts have been cancelled, and who wish to apply for government funding to continue training in the same qualification with their supervising registered training organisation.
